CCofSP is a member of Community Care of North Carolina (CCNC), an organization that helps primary care providers (PCPs) develop the capacity to manage the health care needs of the Medicaid population and to improve the quality of their care by taking a group management approach. CCofSP’s core mission is to work collaboratively with community physicians, hospitals, health departments, and departments of social services to establish local systems that achieve long-term health care quality, efficiency, access, and utilization objectives for Medicaid and the uninsured. The CCofSP network includes Cabarrus, Rowan, and Stanly counties in North Carolina and utilizes a medical home model.
CCofSP is the collaboration of healthcare providers and community agencies to increase the access to and quality of care for Medicaid and dually-enrolled Medicaid/Medicare patients in the three counties. CCofSP’s goal is to improve population health outcomes and to engage patients in their own plan of care. CCofSP achieves this by:

CCofSP physician leaders come together to design and develop clinical improvement initiatives in areas including:
- Asthma Disease Management
- Congestive Heart Failure Disease Management
- Diabetes Disease Management
- Emergency Room Initiatives
- Pharmacy Management Initiatives
- Case Management of High Risk/High Cost Patients
We serve our region’s most vulnerable population and save tax dollars by (1) coordinating and streamlining services, (2) sponsoring health care improvement initiatives and interventions and, (3) improving the overall quality of care for patients through cooperation.
